Medication Management
Medication Reconciliation is a service provided to inpatients at GRMC. Upon discharge, patients receive a list of all their medications and over-the-counter supplements and medications recorded. This information is also stored electronically at GRMC with the medical record.
Taking this list to all healthcare appointments can help bridge communication between providers. It is really important that healthcare providers know what patients are taking, how much they take, and how often they take it. This includes vitamins, supplements, and all non-prescription medications.
To keep the information easily accessible GRMC provides a Red Card holder for your list and insurance card. It is invaluable in an emergency healthcare situation to allow healthcare providers to know all your medications right away.
